Our Billing Practices:
  1. We will gladly send a claim to your insurance company for your treatment at our office.

  2. We will follow up with your insurance company if the claim is delayed in payment.

  3. We will attempt to obtain a satisfactory response from your insurance company about any disputed bills.

  4. We will send you a monthly statement if you have an outstanding balance.

  5. Payment to our office can be made by Cash, Check, Mastercard, Visa or Discover Card.

  6. If your balance becomes overdue, we will contact you by letter to let you know.

  7. If you fail to pay your overdue outstanding balance, and we are ultimately forced to send your account to a collection agency, we will be unable to provide you with further treatment until your account is paid in full. You will be notified of this by certified mail.
Your Financial Responsibility as a Patient:
  1. You are responsible for contacting your insurance company to verify that Dr. Steven Cohen is in your insurance company provider network.

  2. You are responsible for providing us with your most current health insurance information.

  3. You are responsible for the co-payment and deductible portions of your bills as contracted with your insurance company. The co-payment and deductible portions of your charges are due at the time of treatment to the extent possible.

  4. If your insurance company requests information you are responsible for contacting them so that your claim/s can be processed. In this case the bill/s will be released to you, and you will be responsible for payment. We will gladly issue a refund to you if your insurance company subsequently pays the claim/s.

  5. You are responsible for paying your outstanding balance when your statement is received.

  6. You are responsible for contacting our office if you are unable to pay the entire balance in full when you receive your statement. A payment plan can be set up, and you will be responsible for making substantial monthly payments toward your balance due.
